Authorization For Release Of Information 

This form, signed by the student, confirms the student's consent to the release of information (as specified by the student) to the person(s) listed by the student.

Dependency Statement
This form is used for parents seeking information about their student and is meant to prove the student's dependency.

Academic Improvement Plan (AIP)

The online questionnaire is accessed at: http://aip.byu.edu

Students on academic Warning or Probation must complete this questionnaire and review it with an advisor and/or faculty member in their major.

  1. Complete the online questionnaire at: http://aip.byu.edu.
  2. Make an appointment with your College Advisement Center advisor and/or faculty member. To find out who your faculty member is, contact your College Advisement Center. Click here for a list of the College Advisement Centers.
  3. Meet with the advisor and/or faculty member to discuss the plan. He or she will approve the plan and submit it to the Academic Support Office.
  4. They may require you to return for further help and advisement.

Graduation plan for returning suspended students

Returning Suspended students must complete this packet after being readmitted through the Admissions Office to have their academic registration block removed. The Academic Support Office will contact you to ask if you would like to pick up the packet or have it mailed to you. To complete this packet, complete the following steps:

  1. Complete the AIP (http://aip.byu.edu) and graduation plan (to the best of your ability).
  2. Make an appointment with your College Advisement Center advisor and/or faculty member. To find out who your faculty member is, contact your College Advisement Center. Click here for a list of the College Advisement Centers.
  3. Take the draft of your graduation plan to the meeting with your advisor/faculty member for them to review and endorse both the AIP and the graduation plan.
  4. They may require you to return for further help and advisement.
  5. Make an appointment to meet with an Academic Support Office advisor (2500 WSC, 801-422-2723).
  6. Take the signed graduation plan to the meeting with the Academic Support Office advisor. They will review the documents and complete a contract with you explaining the expectations required and resources available to you during your first semester back at BYU.
